Passport Hours

By Appointment Only


General Requirements

  • All applicants must appear in person at time of application.
  • Applications for Children 16 and 17 years of age require the presence and signature of one parent.
Applications for children under 16 require the presence and signature of both parents. If the second parent is not available to sign, the appearing parent must present evidence that he/she has:
  • sole legal custody of the child, or
  • notarized written consent of the other parent for the issuance of the passport Form DS-3053, or
  • written statement explaining why non-applying parent's consent cannot be obtained.

Required Items

  • Completed Application
  • Evidence of Citizenship which may include: Certified Copy of Birth Certificate, a Previous Passport, or Naturalization papers (must be submitted with application)
  • If a child under 16 has a previous passport, additional Evidence of Citizenship is still required.
  • Social Security Number
  • 1 color passport photo - must be taken within the last six months
  • Proof of Identity - valid driver license, government, military, or state ID, or previous passport
  • Check or Money Order for each application -the Department of State requires their fees to be in check or money order.

Fees


All fees are regulated by the US Department of State - Bureau of Consular Affairs.  These fees, payable to the US Department of State are mailed with the application and must be in the form of a check or money order for each individual application.  We can generate a check for you for an additional fee of $4.00 for each application.

In addition to the fees listed below, each passport application has a $35.00 acceptance fee, payable to the Davis County Clerk.

Passport photos are provided at this location for $10.00.

 

Passport Book - valid for all land, air and sea travel

     Applicant 16 and older:
          $130.00 - for regular processing (6-8 weeks)
          $190.00 - for expedited processing (2-3 weeks)
          $190.00 plus current overnight mail charges - for expedited and overnight mailing
     Applicants 15 and younger:
          $100.00 - for regular processing (6-8 weeks)
          $160.00 - for expedited processing (2-3 weeks)
          $160.00 plus current overnight mail charges - for expedited and overnight mailing

 

Passport Card - valid for land border crossing or sea ports of entry from Canada, Mexico, Caribbean and Bermuda

     Applicants 16 and older:
          $30.00 - for regular processing (6-8 weeks)
          $90.00 - for expedited processing (2-3 weeks)
     Applicants 15 and younger:
          $15.00 - for regular processing (6-8 weeks)
          $75.00- for expedited processing (2-3 weeks)
