Acceptable Identification List to Obtain Vital Records

ID MUST BE CURRENT

Identification is required for all non-public Vital Records. Mailed requests must include an easily identifiable photocopy of the back and front of your identification from the list below. If no identification is enclosed, your application will be returned.

If you cannot provide acceptable identification, you may have a spouse, parent, grandparent, sibling, or adult child who can provide appropriate identification to request the vital record.

The State of Utah requires proof of relationship

Birth/Stillbirth Certificate

  • Birth certificates are available approximately 10 days after the child is born.
  • Birth certificates for births anywhere in the state of Utah that have occurred from 1926 to the present may be obtained here.
  • Utah County birth certificates are also available through this office back to 1882.
  • Utah birth records become public after 100 years of the birth event through the State of Utah Archives.

  • For those born in Utah but currently live out of state, call (801) 525-5150 for questions OR go to Utah Vital Records to order online.

Fees

  • $22 for a Birth Certificate, $10 for an additional certified copy that is requested for the same record on the same day
  • $18 for a Stillborn Birth Certificate

Certificate Requests

Requests require the applicant to be the individual or an immediate family member (child, current spouse, parent, sibling, grandparent, grandchild) of the person whose record is being requested. (See Utah Code 26B-8-125) The State of Utah requires proof of relationship.

  • Birth certificate requests can be made either in person or through the mail.
  • Acceptable identification must be presented when applying for a vital record.
  • Requests must include the appropriate fee.

Voluntary Declarations of Paternity

The legal process used to add a biological father to his child's birth certificate when the parents are not married at the time of birth. The mother must initiate the request. The process requires submitting a Birth Certificate Application completed by the mother, completed Father's Worksheet, a copy of the parents ID, and the correct fees. The declaration is then sent to the State to amend the birth certificate.

Affidavits for Correction

The legal process to correct minor errors that may exist on the birth certificate. This process requires notarizing signatures and submitting the documents to the State for correction. For minor children, both parents need to sign.

* If father is not listed on the certificate, the mother and one of her immediate family members must come in.

Death Certificate

The Office of Vital Records and Statistics has records for deaths that occurred in the State of Utah from 1905 to present . Utah Death records may be obtained here.
Utah death records become public after 50 years of the death event through the State of Utah Archives.

  • $30 for a Death Certificate, $10 for an additional certified copy that is requested for the same record on the same day

We issue Utah marriage and divorce certificates that occurred between 1978 and 2010.

NOTE:
If you need a marriage record for a marriage that happened outside of 1978-2010, please contact the county clerk office that it was filed with.

If you need a divorce record for a divorce that happened outside of 1978-2010 or you want the decree, please contact the district court that it was filed with.
