Disease reporting

Disease reporting is available 24 hours a day, 7 days a week. Calls made after-hours or during weekends and holidays will be sent to a confidential voicemail system. This reporting line also allows for emergency calls to be forwarded to an on-call staff member. All messages will receive a prompt response. Relevant laboratory results should accompany a disease report. Disease reports and/or laboratory results should be faxed to (801) 525-5210.
