Personal Immunization Records Requests

We are able to provide personal immunization records for most participants who have received shots at the Davis County Health Department or whose medical providers participate in the Utah Statewide Information System at no cost. We are also able to fax records to a doctor's office or school at no cost. Immunization records will only be provided to the recipient or his/her guardian.
